You can sign up for a house call below. Or, you can refer a friend to us at the bottom of the page. If you wish, you can download new patient forms below, too. Of course, we can bring forms with us to your first visit.

Request a Housecall

Our on-line housecall request will pop up when you click on the photo or on "Open". One of our scheduling specialists will then contact you to schedule DRS' first visit to your home. Or, if you prefer, call us toll free at 1-877-423-6286. Our local number in the Metro-Detroit area is 248-356-5534.

Patient Registration

DRS can usually arrange your first appointment with one of our practitioners within 48 hours of a request. Submit your "Request A Housecall" form above or call us at (877) 423-6286.

New Patient Documents

Download new patient forms and HIPAA privacy forms here. You can have these ready when the doctor arrives for your appointment. If you prefer, our Medical Technician will bring the forms with the doctor to your home.
